The Mission of 1 True Health – Care Management is to improve health outcomes and overall health and wellness for the 40% of the U.S. population suffering from one our more chronic or acute diseases.
Our Vision is to empower and educate an individual (or their Caregiver) to manage his/her health through compassionate and personalized Chronic Care Management and Remote Patient Monitoring services. We help the individual to better engage in a coordinated Care Plan and we provide fun and engaging health & wellness coaching.
We also protect the individual from future health threats by identifying the signs and symptoms that indicate vulnerabilities to infectious diseases and reveal new chronic or acute conditions. And we help the individual to better connect with their Physicians.
